Creation of a Short Film : Release Forms...

When creating a film it is essential to have gained the correct permission. A release form is needed for anyone who is either interviewed within a documentary or speaks on camera. Therefore this meant that I needed to gain consent from my actress Hannah and my my actor Edward but would not need to gain consent from the general public as they are not focussed on within the film, however if I was using a sensitive location (example = a hospital) I would need to gain consent from those who were identifiable within the film. Although this is not appliable to me as both my characters are above 18, if I was using anyone under 16 I would also need to gain consent from their parents. When I handed out the release forms, I went through the true nature of the film to ensure that the consent I gained was fully informed and that they were not misled, I also handed out a copy of the term and conditions which I found from a source on the internet explaining about what they are signing up to and their rights ect.  I have photographed evidence of the full consent form I handed out, two close up shots of the actor and actresses signiture and a photograph of the term and conditions.
